We present a patient who came to our clinic presenting a series of significant challenges in her nasal structure. The asymmetries were notable, with a caudal septal deviation affecting the midline of her face. Added to this was a significant mixed osteocartilaginous hump with a low radix, which contributed to a poorly defined profile. The nasal tip appeared droopy due to weak cartilages, accompanied by a hanging columella without support, elements that negatively impacted the harmony of her midface.
